1. Memories Traverse City Liability Waiver###

Please complete the liability waiver included in your confirmation email above with the button.

2. Polaris Rider Check-In Form — REQUIRED: Located below###

This is a separate form from the liability waiver. Every adult—drivers AND passengers—must complete this form too. You will be navigated to Polaris to complete their check-in evaluation.
